I may be wrong, but I believe that you are not eligible assistance if you own a home. Disabled or not. Actually, I don't think assistance to make mortgage payments exist. Assistance with rent, yes, mortgage, no.

When you purchase homes they offer you insurance through the mortgage company in case something were to happen that you could not make your payments due to illness or injury. They make the payments for you if you had you gotten it.

This is a tricky area and depends some on where you live. You may be eligible for a Section 8 housing subsidy that can be used to pay a monthly mortgage. The 3rd section down on the following web site has information about assistance with a mortgage: https://s044a90.ssa.gov/apps12/best/benefits/

I hope this is helpful. $1500 is not a lot to live on.

Why would that entitle you to any help? If you are a disabled veteran that will get you further exemptions on your homestead in some states. But the mortage is your responsibility. Sorry.
