Is there any formaliy I should follow when writing such document?

Are you talking as the person writing the statement for an handbook or are you talking about as a victim or witness of sexual harrassment.

If you are talking about writing a statement, I would do a search on google on "sexual harrassment policies" and see what comes up. There are some very good statements out there. In fact, here's a link I saved in my bookmarks: http://www.entwistle-law.com/law_firm/sexual-harassment/nine-steps.htm


Now if you are talking as a victim or witness, think in terms of
Who
What
When
Where
Why

Did the accused make any specific comments, did they specifically touch any the victim any place on his/her body, the accuser's facial expressions, accuser's posture, did he/she appear threatening or intimidating? You want to stick to facts as much as possible.

At the end, you can also list your feelings on the issue, but keep these separate from the details. This is not as important, though as the details, because the legal prosecution has to be based on facts, not emotions.

Write exactly what happened. Be 100% truthful.
Use the exact words said, if you remember them.
Write down the names of any witnesses.
Include dates, times, and places of occurrences.

It depends on the context. First, you would say where it happened, how long (over what period), what was said, what was done, etc. It would also be good to mention whether this behavior was done to you, exclusively, or whether this was also done to others. If it was done to others, you might try to get a supporting statement from them. Basically, you must try to be as exact as possible: remembering dates, exact words, who saw you, etc.
