I'm not sure about this but with Steve Irwin's death, they said that the video showed him pulling the barb out of his chest. They've been showing barbs all morning so I'm thinking that it must detach.

No it doesn't. Sting ray barb is poisonous but its poison is barely lethal., though highly painful. The fact that it struck Steve Irwine in the chest and pierced his heart is the cause of his death.
