you could also ask how can you see dreams when your eyes are closed when you're asleep, maybe the brain is using its past experience to provide the "sound and pictures" somehow

It is impossible for me to say with absolute truth, but when most people think inside their heads, they think in a language. For instance if I am planning something, such as a day, inside my head will be something like: 'So I'll wake up at about 9, make pancakes and yeah I can use the honey and butter to make a sauce for it' all in plain English.

If there are distractions around it becomes increasingly difficult to keep track of these thoughts, and one of the most distracting things are loud noises. Hence 'Its so loud I cant hear myself think', a noise so loud it drowns out the words of your own cognition.

In real terms you don't hear yourself think. You just think. Unless you transform a thought into a verbal utterance like I just did when I knocked over my coffee and thought "s**t!", the thought is not audible. However, deliberately saying your thoughts aloud can help you concentrate because you use additional parts of the brain and therefore form more memory traces.

"Hmmm, interesting answer", I just heard you think :-)

you don't hear it...
you only hear something that is said aloud, therefore creating vibrations that are picked up by your ear and registered in the brain as sound.
You just KNOW what you are thinking!

by actually verbalizing your thoughts, people around might think you're crazy and talking to yourself, but thanks to things like ipods, mobile phones etc just put an earphone in your ear and people will think you're either on the phone or singing along to music
