shes paranoid,abusing her children everytime anger builds,ruins their personal things, poor personal hygiene,she nags,she shouts,
my friend is really tired and sick living with his wife.

Divorce and get custody of the children.
Try and get her to doctors or marriage counselling first, to start building up picture of her mental attitude etc, so if divorce is necessary you outside professionals backing up your friends claims.
Sounds like she has very bad depression, either clinical or manic, but only a doctor can asses her for this.
Have other family members witnessed her problems or commented on them?

Divorce her, but have plenty of proof that she is a nutcase. Get custody of the kids. Move to a different state.
If she is abusing the kids, he needs to get the police involved.
