I believe that Ichi died but Takeshi (the little boy) didn't. You saw him at the end so he can't have died. Before Kakihara deafened himself he said "Damn...no one left to kill me", so after he burst his ear drums he hallucinated or imagined how Ichi would kill him, you may have noticed that it went quiet after Kakihara did this. He imagines how he wants Ichi to kill him (Ichi holding Takeshi's head, Ichi slicing Kakihara's head with his boot), that was all Kakihara's imagination, Kakihara must've threw himself off the building. And that's when you see Jiji move his hair and not see the scar, because Ichi didn't really do it. There are so many different interpretations for this movie's ending but that's mine.
Reasons why Jiji was hanged/hanged himself:
He only wanted Kakihara dead, so now that it had happened he hanged himself.
Takeshi wanted to make like Ichi and rid the world of bullies and so hanged Jiji as he was the real bully.
Takeshi hanged Jiji because he didn't want him to hypnotise his mind into killing people.
Jiji saw Ichi dead, his plans were ruined so hanged himself.
Jiji saw Kaneko dead (remember he told Ichi that Kaneko was his long lost brother, this must've meant he didn't want Kaneko dead) and hanged himself.
And that's when we see Takeshi older, as the 'new Ichi' if you will. Oh I forgot, I think Ichi died because he simply bled to death from the bullet wound in his leg. I'm not sure what happened to Takeshi, I'm guessing he just ran off after seeing Ichi dead. The only thing I don't get about the ending is when you see Kakihara in the blue room sitting in the bathtub, and his jaw drops, what do you think that's about?
