There is an awesome product called OfficeReady that I use. Check Ebay. It not only has invoice template but statements, purchase orders, business cards, labels, envelopes, etc. I think I paid $4-5 for it. OfficeReady is by Cannon Software. In addition to using Word, the templates also use Excel and PowerPoint. Check it out!!!

Microsoft Office and Lotus smartsuite has some good invoices included.
There are a ton of books out there on the subject. Head off to the bookshop to browse for ideas.
A lot of business men I know who do what you do just use an invoice book that they purchased in an office supplies shop along with a stamp of their business name and address.

Not sure what CIS means. It's easy to do in Word.
Do you have a logo and company slogan? You can cut and paste that, put in your address, telephone number, etc, label it Invoice and insert table, adjust columns, fill in headings etc and Bob's your Uncle.
It depends on your proficiency in Word.

Microsoft Excel has some in built ones. You can also check online.
