A 12 month excuse is really all you need right now. Fill out the form and check the box that provides for the medical excuse. If he gets another summons next year, do the same. If they request more medical proof, then supply what they require. It really shouldn't be a time-consuming problem for you to take it a step at a time, a year at a time.
In my very populated area, a person may go years before getting another jury summons.

You can ask the Commissioner of Jurors what they need to pull him from the jury pool, normally a simple note from a doctor will do it. They are actually pretty nice to work with and understand this as many seniors get jury duty notice and their children have to contact the office.

Return the form assigning him to jury duty with a statement that he is ill and unable to serve. You should receive a reply excusing him from duty.

Have him show and then let the county take care of him. It is a good question. They wanted me for jury duty in Oregon, the road was washed out and I had no transportation. They threatened to have a sheriff pick me up. I finally called and told them I was a criminal.

Simply have your doctor fill out a form "I'm sure he'll know what one" and turn it in at the courthouse. He will be excused for Medical reasons No Problem. I terribly sorry about your father.

I have really bad arthritis in my hips and can't sit for extended periods of time. I had my doctor write a letter and that was that. It excused me for good.

Have his doctor write a letter and send it to the court with a cover letter from you explaining it.
If he gets called again, repeat.
