If you do, you can most likely get it at the airport when you enter Muscat. I recently went DC > Amsterdam > Egypt and I bought my visa when I arrived at Cairo International.

Very Doubtful.
Most countries have a transit area where connecting passengers can stay between flights. Ussualy it is considered an international zone, so you technically are not in Oman. If you leave the airport you would need a transit visa. Staying in the airport would not require a visa.
Best advice would be to double-check with the airline you are flying with. I would say there is a less then 2% chance you would need one though.

No you are a transit passenger. You do need one for Nepal.
NEPAL - *Passport and visa required. Tourist visa extendible to a maximum period of 150 days in one visa year (January to December). Single-entry visa for 60 days, $30; double-entry visa for 60 days, $55; triple entry visa for 60 days, $70; and multiple-entry visa with a validity for 60 days, $90; requires 1 application form and 1 photo. Entry visas are valid for entry into Nepal within six months from the date of issue. Tourist visa can also be obtained from Immigration Office at Kathmandu Airport and other specified ports of entry. Airport tax of Rs. 900 must be paid for all international flights into Nepal. Generally if you are in transit a visa is not required, but if you are in doubt contact the Oman consulate and ask them.

Id depends on your citizenship if you are a u.s. citizen you don't need one if:
1. you are holding tkts for next destination
2. you are arriving from Dubai
3. you are holding a stamped visa from qutar, oman
