You will need transcripts from your undergraduate degree, you will need to fill out an application, if you are able you will need to prove residency in the state you wish to get your degree (this will lessen the cost of tuition) and you will need to take the Graduate Record Exams (GRE). If you are from a country other than the US I know you will need a VISA, but I'm not sure what else. The website of the university you wish to attend will tell you what you will need.
