Meet the booking office supervisor where you purchased your ticket. Since you have not yet travelled, you cannot contact "any railway office". You must contact the place from where you say you bought your original ticket from. They will issue duplicate ticket after verifying your identity and collection of 25% fare (I think)

Immeadiately go to the nearest railway junction and contact the officials. You provide all the info you had provided while booking. Also tell them the things contained in the ticket if you know.

You do not need PNR number. Report it to the nearest Railway booking office so that no one can get refund. after that Railway will issue new tickets after prescribed charge.
