Nape piercings are uauially not very painful, but can get snagged on clothing.
Because they are behind your neck, they are a little more difficult to clean, so you just have to be careful.
I reccomend getting pierced with flexable PTFE, which is a flexable self-threading nylon that sits underneath your skin replacing the regular long surface bar. It moves with your body and can help reduce premature rejection. Many people find them more comfortable too.
But keep in mind rejection may and could happen.
Also some people cant even get it done, if they dont have enough skin back there, you wont know untill a piercer checks you out.
Good luck.

my husband has 3 on the back of his neck, 1 horizontal and 2 diagonal. I personally would prefer a surface bar due to the fact that a 90 degree angle exiting the skin would put a lot less pressure on the healing piercing than ptfe or tygon...those both move great but if placed properly there shouldn't be that much movement, and making sure the lifts on the surface bar arent to high it seems to heal better..but then every person is different. If you notice any rejection remove it as this can cause sever scaring in that area

Its called a nape peircing. Nape peircings are surface peircings which make the body sometimes reject the peircing just like other surface peircings. Wearing certain types of clothing that rubs up against your neck alot will affect the area causing it to get infected. They take between 2 - 6 months depending on how well you take care of it.

That sounds horribly painful. How would you sleep comfortably, or put tops on without pulling it. Oww!
