Groupthink occurs when someone in a leadership position surrounds him/herself with people who are "yes-men." They do not question the leader's decisions or offer alternative viewpoints. Even if they attempt to, they are squashed and ultimately pushed out of the group if they do not stop.

The way to avoid this is to encourage people to think for themselves and to play "devil's advocate" with a proposed decision. To deliberately look for reasons that another decision would be better. To openly critique the proposal. Then after informed debate, if the decision stands and survives the critique, fine. But if another alternative comes up better, then all will be better for the process.
