Your City Hall should have that on record.

Contact your state's Board Equalization. You can check the information in the government pages in the front of your telephone book or find it on line by typing "State Board of Equalization" into your search line and on the listings page click on the one for your state. The state is abbreviated in the green web address at the bottom of each listing, but click on the bold blue type, that heads the one you want, to go there.

Contacting your city hall will get you the one you had with the city. I don't know which one you are after or what your need for it is. But, if you need both, contact both places. The one with the state is probably the one of more importance

I owned a business for twenty years and had a state permit. I had a permit with the county, not with the city, because I was just outside the city limits.
