A very general answer for this question is to maintain sterility. But what exactly is the technical reason behind the positive air pressure? And how does it help maintain sterility?

from my nursing textbook: Several methods are used to prevent the transmission of infection. Filters and controlled airflow in the ventilating systems provide dust control. Positive air pressure in the rooms prevents air from entering the OR from the halls and corridors.

In other words, so germs don't flow in to the OR and contaminate everything. sterility of the environment is important. people go in and out through the doors during surgery. surgical wound contamination would be possible, as well as contamination of instruments and materials.

Also. the OR is kept cool (68-75 degrees) and the humidity is regulated to be less than 50%, another environment unfavorable for bugs.

There are both types. In either case, the objective is to restrict the flow of pathogens. Negative pressure would extract air from the room and heavily filter it before discharging outside. It would be replaced by clean, filtered air. This would be used for a patient with a contagious disease to limit its travel and potential transmission to other patients. Positive pressure would be used to prevent a patient from being exposed to any other pathogens and more likely used for an injured patient at risk for infection but without a contagious disease. Either way, the patient is always exposed to filtered air. A positive pressure environment may not require filtering of discharged air.
