If you do not pay National Insurance or are resident in Uk you will be treated for emergency problems free of charge. Anything else you have to pay for. eg, if in a car crash or have heart attack you will be treated in emergency department but if you need to stay in hospital you will be billed.
In practise doctors treat everyone and leave administrators to deal with payment........it usually takes a while for them to find out you exsist

If you don't have travel insurance, yes you will have to pay. The UK has universal health care, but only for it's citizens.
