You mean "Notary Public" forms. A notary can verify any form, there is no limited set of forms they can notarize (verify).

To become a notary public? Check your local Secretary of State, or your local county courthouse.

To employ the services of a notary public? Many banks have them, attorney's offices, some large supermarkets. Or check the phone book.
