It is not considered clean nor sanitary, but it may be the only thing to do in an emergency, or at least to feel cleaner and fresher...example...after almost 30 hours of plane delays (not to mention as to when you'd actually dressed the day you started out) and then finally being put up in a hotel at 1:30 am in a foreign country, with no luggage - and nowhere to buy anything but a cup of coffee, wouldn't you rather do this when you got up at 5:30 am to go catch your plane, and knowing you'd arrive 8 hours later at your destination -- finally! As stated, only in an emergency...
(Note to all who said: 'eeeew' 'yuk' 'that's gross' - I hope this never happens to you!)

Eww, gross! No! No way your mama said "Make sure you wear clean underwear. Or at least turn your dirty one inside-out." That is yucky - wash your drawers!

no because the dirtiness(or the bad bacteria) goes through both ways.. when you use your underwear btoh sides would be dirty....... its like droping juice on your shirt by accident.. the juice goes through the outside and then through the inside.. so when you try to wear it inside out.. it will still be dirty(cuz you will still see the juice stain

Ummm........NO! All the germs and bacteria are on both sides of your underpants sweat and body fluid get inside the weaves of fabric.
