Step 1 would be to get a nursing degree. Then focus on obstetric nursing. Although I do not like the quality of care the Kaiser Hospitals offer, I know that they are looking for midwives in order to cut the costs of using doctors, so a call to their nursing personnel department should give you good advice on the kind of training a Kaiser hospital considers necessary.
If you don't want to become a nurse--and that's where the money is--then speak to a hospital personnel person, or to the office manager of a large medical group. Ask them what they consider good training for being an assistant.
Please do NOT enroll in one of these colleges that offer short courses they claim will prepare you to work in a doctor's office---until you have spoken to personnel or office managers.

Most often you must become a nurse first, then go on to grad school and specialize in midwifery. Most nursing grad schools require you to work in the field for at least one year before you can start grad school. Get a bachelors in nursing, then go from there. If you already have a bachelors degree in another field there are schools that offer a fast track nursing degree that you can get in a year. Look into colleges in your area, or go to allnurses.com, they have a list of educational programs.
