i'd see a chiropractor, if possible. there are too many causes for chronic pain in the neck for any fast answers here. a few things that come to mind: dislocation of disk in the spine, pulled muscle in the upper back, tmj dysfunction (problems with jaw joint). all would require different treatments but a chiropractor can help alleviate the symptoms and let u know the origin of the pain.
