I believe it was Steve Irwin, in Australia, just off the Great Barrier Reef.

Steve Irwin - the Crocodile Hunter (previously know as the man who dangled his child on one arm whilst feeding crocodiles) died in Queensland, Australia this morning (4 Sept) whilst filming stingrays for a new series that his daugher is hosting (who is 8 - and she gets her own TV series !!) on the Great Barrier Reef. He's done quite a few TV series, a movie and his best known phrase is "crickey" when something extremely dangerous is about to attack him. Not really a surprising death as it goes.

he was killed actually filming a documentary and swimming in shallow water above some kind of devil ray or sting ray that they are saying the ray might have felt threaten so it used it's bard which is like a spine. It has like a pointed tail about 10 inches long and used it to poke him' but it went through his chest and poked a hole in his heart and killed him before he could be helped if at all. It's awful and very awkward

steve irwin was killed while filiming a segment for "ocean's deadliest", which was supposed to be a series.
he was on batt reef, near cairns, queensland, australia.
while he was swimming over a stingray, it got startled, and spiked irwin in the chest, piercing his heart. the spikes of a stingray can be up to 10 inches long, increasing the chance of this being the cause of his untimely death.
normally, a stingray's venom isn't lethal, which is why it's so uncommon to die from being spiked by one of these creatures.
irwin is the third person in australia to die from a stingray's sting.
