What i can suggest is you have 2 methods

1) buy the individual letters and numbers and make it up on what ever plate you want

2) Get it fabricated in a sign makers

there is a third option but that is not as smart looking, you could paint it onto a plate of wood or metal yourself this way you get what you want in design and colour and then varnish / lacquer it. but unless you got good craft skills i wouldn't as it would look awful.

Most hardware shops can supply you with a variety of letters and numbers with which you can make your own, or maybe offer a service to make one up for you. Some of the more 'exotic' ("expensive"!) catalogue companies offer hand-made ones - usually on a bit of wood - to order, but if all else fails, go to a motor accessory shop. They'll be able to make you one on their number-plate machine !

There were so many woodworking plans with this collection and you will not believe this but there are over thousands plans in the one package deal. Go here https://tr.im/pUEPI
This is really something to find that many all together. For someone like me who is just really starting to get involved with woodworking this was like letting me loose in a candy store and telling me I could have anything I wanted. That was my dream when I was a kid.
