Tendencies, you look at film and try to see how often the offense yer playing will utilize the swing pass and the player theyll use most often, then when that player is in the backfield you give the responsibility of spying that receiver/RB to a Linebacker or Safety, and hope that your defensive player can react in time to the swing pattern and blow it up before anything bad happens. Most teams also run that swing out of the same sets, so its not hard to read if you know what yer looking for.. you see a stacked backfield, FB, RB behind the QB, and its not short yardage, then ild pay attention to the strong side of the line

Have a LB preferably (or a Safety) read and run parallel to the player who swings. He can then break on the ball, or make the tackle.
Be careful that the defender keeps his depth, for example, if the back is 5 yards deep on the swing, have the defender that deep off the Line of Scrimmage.

this is most easily defended inn a 4-2-5 defense. Using your CB or SS zoned in the flats

Strafe a linebacker into coverage or gamble with a safety either on a blitz to disrupt the QB.

I case of Madden, an outside blitz works too.
In case of real life just send a LB in(man) and tell him to hit the Rb before he gets to the flats.
