You can also download passport application form with complete detailed rules from http://passport.nic.in and can actually use this form for making formal application for a passport. Also, fill the form completely and truly with all support documents and fee in first go itself for fast and quick favourable disposal of your application. Due to this, I have the benefit of getting passport(s) in straight 4-5 weeks time in normal course. Do read the rules first and fill the form complying everything asked there.

For Visa, every country issues its own visa to visit their land (with exception of certain European Union Member Countries) and have their own set of rules and fee structure for issuing different visas. So whichever country you wish to visit, consult their respective rules by either visiting their Embassy / High Commission or their respective websites for such information. But a formal application is required in each case, alongwith their prescribed fee.

Passport - You should go to your passport office and fill out an application form, an authenticated copy of your birth certificate and passport fee.

Visa - you go to embassy the country where you wish to go. There are certain requirements that the embassy required. All countries have different procedure in applying a visa, also what kind of visa you wish to have tourist, business or student.

If you are an Indian Citizen then the procedure is as follows -

1.Go to regional Passport office in your area and obtain application form for passport. May be It can be obtained in your local post office also, pl check it !

2. Fill it with all details and attach attested copies -
- For the your age proof [ Birth certificate, School Leaving Certificate ],
-12 passport size photos [35 mm size ] with proper background so that whole photo can be seen clearly.
- If you are married -Marrige Registration certificate.
- Residence Proof-Ration Card, Light Bill, Tax Bill etc.
- A certificate from society's President, where you are living,
regarding since how you are living in the house.

3. then submit it with Rs.1000 Registration fees at Passport Regional Office.

4. Wait for Police Inquiry. Where you have to produce two witnesses for your character checks.

5. You can log on http://passport.nic.in for the status check up by inserting your application receipt number.

But if you dont want to wait in q's then go to your city Head Post Office.Ask for Passport application.They would tell u the details.U will need a DD for rs.1000/- favoring "The Passport Officer"-verify with the postal authorities.Fill up Appln,Take 2 photocopies with Photos and affix 2 photos in original,1 each in photocopy.U will get ur passport by SPEED POST to home.

Call your local post office,you can apply at most post offices.You'll need your birth certificate and drivers license.They will take your picture when you apply.When your passport arrives,contact the consulate of the country you intend to visit,they will supply the visa!

In Canada you can pick up an application at most Post Offices.
They have all the instructions in them.
