I'm pretty sure it's an analogy; teeth don't have skin so if you're getting them to hold on for you with their skin, you're pretty damn lucky. Also, I hear U.S. English has the most ridiculous sayings of all languages, making it the hardest to learn thoroughly. Crazy freakin Americans.
