I don't know where this originated from but it's an old saying that means you are barely hanging on to life or what ever. Like just making it,at the end of the rope so to speak.

No there is no skin on your teeth, it is simply just one of those saying. It means that you where soooooooooo close to something bad like failure or death.

I'm pretty sure it's an analogy; teeth don't have skin so if you're getting them to hold on for you with their skin, you're pretty damn lucky. Also, I hear U.S. English has the most ridiculous sayings of all languages, making it the hardest to learn thoroughly. Crazy freakin Americans.
