I would tell her.
I was in that situation. This old, old lady in a restaurant left the ladies room right before I did. She tucked her skirt in her hose. I told her and she fixed it and rushed away. We were seated near each other and she said "I guess it's old age." I said "No, Ma'am. I did the same thing yesterday."
I've been wearing thigh his since then.

Yes I would. I told a girl the other day that her bra was showing... and the week before I told this poor girl had a ginormous line of toilet paper hanging off her & she had no idea...
