If you're talking about the 'UFO' during the Apollo 11 mission, it was part of their rocket, one of the four panels that had housed the lunar lander.
The panels are discarded when they pull the lander out of the rocket, but they continue along the same path as the spaceship.

Are you speaking of Apollo 13?
They aren't completely sure but it has been suggested that they ran into a small meteorite storm which punchured the hull and almost killed the crew
