At the end of The Return of the King, why does frodo leave the shire and where does he go??

He goes to the Undying Lands where some of the elves were from. Galadriel was from there. I think it was called Arda. Frodo goes because he was a bearer of the ring. Sam goes too but he has to live his life first before he goes. Frodo goes first though because of the great burden he carried and he can no longer go back to the way things were before he left the Shire. It's all a metaphor for how lives were changed for the young men who came back from the war. Read The Silmarillion. It explains alot.

Because after all his adventures, life at the Shire isn't the same as it once was.
As for where he goes, the elves are going to a location outside of Middle Earth. It's an island or something like that. Frodo sails with them to that place.

Frodo is departing Middle Earth with the Elves to go to their home.
This is happening to make the point that those who fight to save their home may not be able to enjoy it, even if they survive the battle. Frodo was so changed by his battle that he could not fully return to The Shire.
Remember, Tolkien's young son was fighting in World War II while Tolkien was writing the original books. Think about all of the soldiers who have come home from wars only to end up homeless or ill in multiple ways and unable to enjoy the home they were told they faught to save.

he leaves the shire to go with the elves and his uncle, he leaves because the stab wound from the first movie had someone how made him not well
