Try this website zappos.com. you can search for your boots.
The sweater looks casual, but nice-try for something similar at A&Fitch, Hollister, even Banana Republic.
Also try some surf brands, they always have nice long sweaters. Billabong, Roxy, Lilu, etc.

The boots look like Aldo or Nine West.
The sweater looks like something I've seen at J. Crew and Beneton.

That outfit looks almost exact to the one in last month's J.CREW magazine, the boots look like Donald J. Pliner. Hope this helps, I love the outfit!
