It's a phrase used decades ago, and being (mis-)used in the current instance because it sounds nasty.
A fascist is someone trying to establish a right-wing authoritarian hierarchical government, one opposed to democracy or liberalism. None of the terrorists so described seem to be very interested in establishing a government, right-wing or otherwise. So the term fascist is not accurate.
The attachment of the prefix "Islamo-" (or "Islamic") just associates Islam with the word Fascist in the minds of the audience, and is a not-so-subtle way of implicating the entire religion with deniability.
Amusingly, even Bush acknowledged that "this radical ideology is very different from the religion of Islam" (November 2005).

I think Bush used the term "Islamo-fascism", which is simiilar, although Wikipedia has, for the moment at least, given it a separate definition.
Salafism -- Wahhabism, Deobandism, the Muslim Brotherhood and the teachings of Sayyid Qutb and all the rest related to it -- hold more or less, and in common with Khomeini's teachings -- that democracy is only valid insofar as it arrives at the proper solution: election of an Islamist slate to power.
The intolerance and totalitarian nature of these teachings lead us in the west to see Political Islam as both Islamic and fascist, although technically that's probably an oxymoron. Suffice to say, Bush's listeners know what he is talking about.
More accurate adumbrations of what Islamists (Salafists and their Shia counterparts like Hezbollah) want can be found in the works of Bernard Lewis (Princeton) and Gilles Kepel (Paris).

In contrast to the heirs of some other non-Western traditions, including Hinduism, Shintoism and Buddhism, Islamic societies seem to have found it particularly hard to institutionalise divergences politically: authoritarian government, not to say Islamo-fascism, is the rule rather than the exception from Morocco to Pakistan."

They absolutely are striving to establish a government. It may not be "right-wing" as you understand that confusing and muddled -nearly meaningless - word, but they do intend to re-establish the Caliphate and restore the Islamic empire.
And Bush's statement is only amusing because it is wrong. Islam demand, as a basic tenet of the faith, the establishment of an Islam-based government, even using violent means to achieve that.

1. Al Queda, Hezbollah, Hamas, Taliban are terrorist organizations and they believe and practice the religon of Islam.
2. Fascism: To gain power through violence and ruthlessness.

It means nothing to me actually..its about the same as him calling the iraqis who fight against our invasion being called "insurgents." He got the word from karl rove or some other adviser he sure didnt think of it himself.. he has not proven to me that he can think for himself at all (remember the debates when he had to answer the questions somewhat from his own knowledge)
