If your site supports PHP, then I have an absolutely fabulous script for you.. I use it on my website and it works fine. You don't have to edit anything. The best part is that you don't need PHP knowledge. Data is stored in a FLAT FILE.

Read the "README" file to know how to install it... The guestbook integrates with your site easily.
If you are interested in the above guestbook script, you can always go in for remotely hosted guestbooks. I recommend http://www.dreambook.com.
Dreambook offers themes and it is customisable.

Just buy the the quest book pages or find free ones, customize and link it to the website.
