where it was introduced that when deciding between teams tied in a qualifying position, results against "minnow" teams such as Andorra or Faroe Islands weren't counted?

It may well have been introduced to discourage bigger teams running up high scores, and "humiliating" them

I agree that a qualifier would be a sensible idea but after the Bertie years and a rather humiliating 2 each draw 4 years ago, would Scotland have been in it?

This has never happened. Every World Cup/European Championship qualifying campaign (and the final stages) has counted all goals scored and conceded in any group.
That said, goal difference hasn't been the only decider in the case of tied points - the previous results between two tied teams has been used as a tiebreaker much like they do in the Champions League. Back in the old days, they would simply arrange a one off playoff match.

Well mate, I don’t think of it in the same way… in fact I think that with this “proposal” from fifa, now the sc. “Minnow” teams are given the chance to mix with teams of such caliber like England or France, and the big teams are not humiliated as you say, but fifa is just looking for rules that does not give the prefix “favorite” to one team but to give the both of the teams the same options (even though we see that that’s not quite possible, and there always will be a huge difference between teams like England and teams like Andorra)… but anyway why would you like those results to be counted? It’s better this way, don’t you think?

no, they were all counted. i'm pretty sure in the last world cup qualifiers the czech republic beat andorra 8-0 so i don't see how uefa are trying to save their embarrassment
personally i think all the 'minnow' teams should be put into one group, and either one goes through to the finals, or two go the play-offs

-liechtenstein
-andorra
-san marino
-malta
-faroe islands
-cyprus or northern ireland
it would give them a chance, right? most of these players aren't professional but it doesn't mean their countries shouldn't be fairly represented

Not that I'm aware of. Sometimes if teams are level, they use the aggregate score of the 2 games between those teams (as if it were a 2 leg cup tie with away goals rule etc) within the group as a deciding factor. I'm not sure if this was used as priority over goal difference or only if goal difference and goals scored (against anyone) were idential.
They do like to change things from tourament to tourament.
