"How to prepare Questionare for motivational profile of doctors using herzberg's motivation theory"

I have to do a project for profiling hospital staff( doctor, Nurses, Medical technicians) using herzberg's principle of motivation. can any body help me with it. If some body has done some work in this field please help me.
