To me, it means the person is asking how many people are really living their lives to the fullest. Are you appreciating being alive, loving the people around you, fulfilling your dreams, being who you want to be, caring about/for your fellow human beings, loving yourself, loving your God...or are you asleep and walking through life with blinders on?
