I'm not into tatoos (but think they're cool).
The "claddagh ring" came to mind. "The now famous tale, about a townsman kidnapped into slavery, who returns to present a ring to his true love, is one of the most popular romantic tales of Ireland.
...Claddagh rings are a traditional token of loyalty and friendship as well as romantic love. ...The hands in the design represent friendship, the heart, love, and the crown, loyalty. ... as a wedding ring, it is worn on the left hand, with the heart pointed inward...." http://altreligion.about.com/library/glossary/symbols/bldefscladdagh.htm
You might want to add a flame in back of the heart design?
Another symbol - cinquefoil aka rose of venus - see a variation at the end of the article - you could put something else in the middle of that pentagram design.

But one thing -- let your wife decide on which one she likes. She has to look at it every day so it should be something she likes plus it would be something to do together.

Try watching something like Miami/LA/London Ink, although you may not see exactly what you want, it will give you some ideas. You will be able to see how tattoo's look on different parts of the body and you will be able to see what areas you like tattooed. It will also give you ideas on how to research tattoo ideas, and approach an artist to make sure you get what you really want.

how about the scales of justice(as in the justice system)? put her in the air on the right side meaning she is the right one (that she has come out on top) with a halo on her head i would put a small pic of just her face and put the love of money (a dollar sign), a woman's body with a grim reaper head, and other dark and evil looking things to represent pain you've caused, lies, deciet, coming our of orange and yellow fire with streaks of blue on the left side on the bottom. a banner in red for love with her name or even better yet, her nickname you call her. another gr8 thing to do would be by her head, put he kids names if you have any.
a tattoo like this i would make sure she is directly over your heart, where she belongs. a tattoo of this meaning and magnitude...she should be with you at the time you get this done and explain the meaning behind each thing and tell her you love her. also, if you have a religion, such as christian, i would make a small cross somewhere to show God blesses your union and keeps it strong.

I was thinking abut two commitment rings linked toghether, with your name and her name on the inside.
Try to think of symbols you like that can be incorpored in that tattoo (for instance, although it might be appropriated in that theme, you could hate heart shapes and appreciate star shapes, or ring shapes...)
Hope it helps!

just don't put her name. i think that is a cardinal rule or something.

how about the words, "i am married"? you may try, "i am married to (your wife's name, here), and i've done her wrong; this is my pennance". the first i might put on my forehead, were i you. others, depending upon your transgressions: "poor impulse control", "i cheated on my wife, don't let me do it, again", "i no longer shall watch football", "i shall learn how to cook my own food and do my own laundry; this i swear by the pain i'm feeling, and the blood i'm shedding, now!". tailor, as needed, to your mistakes.
