When a burrowing grub goes down your ear and into your brain, how does it know where the other ear is, to come out that side?

The do not.
They just eat until they get to the end. That is why some sufferers survive but most do not.
Poe wrote the story about the man who survived sane from the bug eating through his brain, only to discover it was a female and now devoid of eggs.
