Most likely: inadequate medicare coverage. Poor people cannot afford medical insurance premiums so will most likely not be adequately covered. Most other western countries have universal state medicare.

Dumbass mothers who don't seek FREE medical attention offered by every hospital because they will be tested for drugs.
Drug addled mothers who don't care they're pregnant because of their addiction.
Medical technology that keeps unviable babies breathing long enough to be considered born but they cannot survive the impossible odds.

I wouldn't be surprised if obesity has a link. Obese mothers are more likely to have premature infants who are more likely to die.
