Several ways really. There could be an uncontrollable hemorrage when the placenta separates from the uterine wall, infection is another possibility....also some women may develop extremely high blood pressure brought on by eclampsia, which could cause a stroke leading to death. With modern medical care, these are rare.

Hemorrhage still occurs rarely due to atony of the uterus or rupture or tear. Failure of placenta do detach is another cause Infection is now a rare cause of death. A rare embolism of blood clots or amniotic elements occurs. A malignant tumor of products of conception occurs very rarely.
