A friend and I were discussing that same thing about a month ago.. he told me that the saying came about and was made popular by a guy that used to fill in for Howard Stern back in the day... so you know it has sexual overtones in the question. Sorry I cannot think of the guys name, but I was told that when they would have beautiful women on the show he'd always act crazy and ask them that.

It means who takes care of you or who do you belong to. It can be kidding or sexual. Usally there is always a sexual undertone but it is harmless.
