i think it should depend on the position of the toilet paper holder versus where the toilet is. i've had it different ways based on that. right now it's under because the holder is on the wall across from the toilet and there is an air vent almost directly under it, that way paper ain't all over the place...plus if the section u grab is around the back it makes it a little less accessable to a 20 month old who likes to play with things in the bathroom.

yes, it goes over the roll, there tear it off, less germs apt to get on the rest of the roll

To me, it makes a huge difference. I prefer over by far. Over just feels more natural and easier to get to/grab.
