I can write forms my self but, normally all the information will be sent to my e-mail and then I have to reply and if i'm not there to reply they will have to wait uintill I am. So are there any that just do that for me for free? (I highly doubt it so if anyone has one I will definitly pick you as the best answer). However if you don't know anything like that then just tell me one that generats it for you.

There are several downloadable forms. Microsoft even has a special portion of their program dedicated to HTML forms. Hope I helped.
