Take all of your documentation to your social security office. You can find your office by using the office locater at http://www.ssa.gov
You will need verification of your new name and old name with photo ID, like a drivers license. Their is a form you have to fill out, I think you can get that on line as well. You fill out the same form to get a replacement SS card.

I wouldn't even bother, unless it's important to you. I got married 33 years ago, and never got it changed. And divorced 14 years ago, and still never bothered to do anything. They just accepted my new name on my tax returns without question. After all, I wasn't trying to commit fraud of any kind, and I was paying all my taxes.

Go to the SS office and fill out a form, it's simple. Take your paperwork with you for proof.

You have to take all the approriate documentation with you to the federal building and have it redone.
