one 250 mg tab of Keflex(this is the med) by mouth ( po) every 6 hours

Keflex is an antibotic medication, 250mg is the milligrams in each pill, tab is the form the medicine comes in meaning tablet, take orally i believe is the i p o q and every 6 hours is the 6h. Hope this helps you out.

Looks like your physician goofed. i makes no sense in that order, but 1 would. As: 1 by mouth every 6 hours. Unless s/he was using roman numerals, which would be asinine.

the answers you have are correct, one tablet, by mouth every six hours.
the roman numeral "i" is not real standard use these days but i see it often from older doctors.
