Why don't you talk to her and find out why she has been acting that way? Its best to be totally frank in a relationship and to not assume anything as assumption is the mother of all f**k ups...trust me on this one. A heart to heart conversation might save your friendship. She may be going through hard times and doesn't know who to turn to and maybe because she's so wrapped up in her problems, she doesn't have time for you. It happens. So just go right out and ask her. All the best.

Then you're not actually trying to break up a good friendship, you're trying to kill a lukewarm friendship. Here's how to go about it....without talking to your friend about what may be bothering her, assume the worst, jump to conclusions, conspire behind her back, manipulate and plot to hurt her, and when you get the chance, tell her a big fat lie...with a grin on your face, no less. That ought to do it. Remember, "the bigger the smile, the sharper the knife". Sounds like you're off to a great start. (I hope you recognize sarcasm when you hear it; grow up and do nothing of the sort).

Ask her why has she been avoiding you? if she gets funky with that simple question, you have just lost a friend. But you'll gain a new one :)
