kalkidan, uh, I think they meant black metal vocalist as in the genre black metal. *rolls eyes*. Black metal is a dark subset of metal. There are many subgenres in metal like black metal, gothic metal, melodic metal, nu metal and so on. The vocalists for black metal usually have deeper sounding voices which is why I'm guessing this person thinks they are altered.
To answer the question, they normally don't use anything, although people who smoke usually have deeper and more gravelly voices, the type that many in black metal seem to have.
