I have previously downloaded a "DIY" Dissolution kit and I have everything I need, except the Acknowledgement form that is required as part of the petition.

I don't recommend anyone try and do a divorce themselves. If you had appendecitis, would you perform surgery on yourself? Its roughly the same idea.

A dissolution of marriage is a complicated affair, that must be done in a certain way. You did not say what state or province you are in, so there is no way for me (or anyone else on here) to give you the terminology to use for the acknolwedment. Also, I don't quite understand what you mean by Acknowledgement of Legal Representation. If you had legal representation, you wouldn't be doing this in the first place, your attorney would handle it for you.

I recommend that you stop trying to save a few $$, and hire an attorney. IF you and your wife are in agreement on everything, the cost of having an attorney draft the appropriate papers is not going to be very high, and if you have an attorney do it, you are assured that the correct documents will be prepared, and they will be properly presented to the Court.

If you decide to hire an attorney and don't know any, contact your local or state bar association for a referral.
