the answer is no. a good example is irregular galaxies like the large and small magellanic clouds, which are companions to our own galaxy, the milky way. one could even say they don't have a centre, let alone a black hole at the centre.

No. Spiral galaxies generally do-- you can see the pattern made as the stars are all getting sucked down the hole. But globular clusters most probably do not. However, if there are big enough stars, any galaxy is liable to have some black holes in it. Whether the galaxy lasts long enough for the black hole to become the dominant feature of the galaxy is a different matter.

A galaxy usually has a sun in its center. A black hole occurs after a sun has gone supernova. Your answer is no. It may have a black hole or a sun in its center.

As I understand it (from watching the "20/20" special), there are wandering black holes. Who knows if every galaxy has one at its center? They are formed by the final death throes of stars, and that could happen anywhere, not just at the center.

Theory suggests that this is true. It has been born out by observation in several galaxies so far. However, the jury is really still out. More math and better observations are in the works.
