According to my wife its girth,length,and color. She likes them as black as she can get. According to her the blacker the bigger and the better they are.

Girth if you can plug it it doesn't have to bottom out

Both. Wouldn't feel right if it were wide, but not deep, nor would it feel good if it went deep but was thin and lanky, like a finger.
But size DOES matter, no matter what we say out loud, to your face.
