In my experience with them they use them as a cover for over the sheets and in the winter they put big down feathered pillows inside the duvet to keep warm. So use both

I use sheets and a duvet - an idea I got from staying in B&Bs.

The duvet cover does not need changing as often plus in the summer, I can do away with the duvet and just have the sheets.

I also like turning down the sheet over the top of the duvet - it completes the bed somehow!

Use sheets under the duvet - it's more hygienic and you get the feeling of nice crisp white sheets against your face and don't have to change the duvet cover every time you change the rest of the bedlinen as it's not absorbing perspiration.

In my house anyway. I have a duvet on the beds, but lay on top of that in summer with light blankets or sheets over.
