First, u need to find your motivation. Otherwise, u will lose focus and never can reach ur goal. now you have too many things in your mind. First clear your mind. Then ,prioritize your options. Actually, when u clam down and think of it. There are not much things to change. Good health just eat good and sleep early. Get good grades just spend more time on studying and less hanging out. In general, it is your time management. Maybe make a time table and stick with it.

Sometimes a crisis, or even something simple like moving to a new neighborhood will change your life. That is how I gave up smoking. Not always easy to arrange a crisis though. Try going for a holiday and being a different person when you return

focus on ONE goal at a time, if you try to achieve too many at once, you will get angry and discouraged and give up on everything you want to change......i think you need to extend the goal time seriously. Try achieving any goals that are smaller at first that way the easier they are to achieve, well the easier they are to achieve, and therefor gives you confidence to tackle the bigger goals.

write down a list of what you would like others to see in you.
Then write down steps you think would be necessary to facilitate those changes. Then just follow the steps you have written.

make a list of goals you want to accomplish. put it up where you'll see it. figure out a plan to accomplish those goals and then when the school year starts, keep yourself motivated in keeping them up.

I have never change my self I am comfortable with who I am thanks to my mother who has given me self esteem. For you you need to self meditate and make a list of all the thigs that you what to accomplish atainable goals ( realistic and reachable ) apply them and and stick to it good lucj
